Pennsylvania Name Change Petition with Motion to Waive Publication Requirement

Many courts have not yet figured out that trans individuals face more risks of harm and unnecessary public scrutiny than others when they need to correct their legal documents to reflect their true identity by filing for a legal name change. Pennsylvania has made progress, but there are still formal requirements that have not been updated. Fortunately, many courts will adapt and endeavor to protect LGBTQ+ petitioners once they are shown how they can do so.

Please feel free to take these sample forms; there is no guarantee that your county’s court will grant the Motion, and the forms may need to be revised if the standard provisions are not accurate — for example, if you have a judgment against you, you may need to list that judgment and serve the court/creditor/etc. with a copy of your Petition for Name Change.
